Form 2 accounts for all materials, special processes, and functional test processes.

5. (CR) Material or Process Name: This field is used to list all materials and special processes (not all processes) used in the product realization process listed on the parts list, purchasing document, drawing, or DPD. This could be a material name or part name, for example, an extrusion or casting.

For alternatives and substitutions see guidance in form 2, field 13. 

Materials and special process in lower-level specifications do not need to be listed on form 2.
• Customer furnished material is accounted for on Form 2 field 5, and in field 8 note it as Customer Furnished.

6. (CR) Specification Number: For materials such as extrusions and castings used to produce a product, list the part number in this field.

9. (CR) Customer Approval Verification: Your customer is the one who is responsible for flowing down any requirements from the end item customer.

13. (O) Comments: Best practice for material or special process alternatives and substitutions is to show the linkage from the drawing to the allowable alternative.
